PAIA manual
This manual is published under section 51 of the Promotion of Access to Information Act, 2000 (PAIA). It tells you what records we hold and how to request access to them.

2. The Information Regulator's guide
The Information Regulator has published a guide on how to use PAIA. It is available free from the Regulator at JD House, 27 Stiemens Street, Braamfontein, Johannesburg, 2001, by email at enquiries@inforegulator.org.za, or on the Regulator's website.
3. Records we hold

4. Records available without a formal request
These are on this website and need no PAIA request: our terms and conditions, our fees, payment and refunds policy, our privacy policy, our cookie policy and this manual.
A member may request a copy of their own health record directly by emailing info@fidere.co.za. You do not need to use the PAIA process for your own record.
5. How to request a record
- Complete the prescribed request form (Form C under the PAIA Regulations), available from the Information Regulator's website.
- Give enough detail for us to identify the record, and say which right you are seeking to exercise or protect.
- Give an address, and a phone number or email address, in South Africa.
- Send it to the Information Officer at info@fidere.co.za, or by post to the address in section 1.
- We respond within 30 days. That period may be extended by up to a further 30 days where the request is complex, and we will tell you if it is.
6. Fees
A request fee and an access fee may be charged, at the rates prescribed in the PAIA Regulations. A request for your own personal information carries no request fee. We will quote any fee before doing the work, and we may require a deposit where the work is substantial.
7. When we may refuse
PAIA requires or permits refusal in certain cases, including where release would unreasonably disclose someone else's personal information (section 63), breach a duty of confidence (section 65), harm commercial interests (section 64 and 68), or disclose privileged material (section 67). If we refuse, we tell you in writing which section we relied on and how to appeal.
8. If you are unhappy with our decision
You may lodge a complaint with the Information Regulator at PAIAComplaints@inforegulator.org.za, or apply to a court for relief.
